HCBS Options 

 

Opportunity Living operates with two different types of waivers, the ID Waiver (Intellectual Disability) and the BI Waiver (Brain Injury). The following services are provided under these two waivers. 

 

  • Hourly SCL (Supported Community Living) 

  • Hourly CDAC (Consumer Directed Attendant Care) 

  • Daily SCL (Supported Community Living) 

 

Aside from the ID & BI Waiver, we are also expanding our Enhanced Living (EL) Program. For more information about any of the above services, please read below. 

 

Hourly SCL (Supported Community Living) 

Our hourly SCL program is based around the team philosophy that there are goals that the consumers accomplish with the assistance of the provider. These goals can be in the home setting or out in the community. Such tasks range from and include: 

  • Social Skills

  • Shopping

  • Budgeting

  • Exercise

  • Attending Appointments

  • Various other miscellaneous tasks 

 

Hourly CDAC (Consumer Directed Attendant Care)  

Our hourly CDAC program is guided by the consumer about what tasks can be done by the provider. These tasks also vary, but a few examples are listed below: 

  • Dressing

  • Hygiene

  • Bathing

  • Cooking

  • Housekeeping

  • Communication

  • Various other miscellaneous tasks 

 

Daily SCL (Supported Community Living/Waiver Homes)  

We currently have 4 waiver homes in Carroll, Iowa. Our community homes allow individuals to live within a community they choose and are staffed 24/7 for clients with the ID or BI waiver. The homes are staffed and help with daily activities and routines (if needed), such as:

  • Grocery Shopping

  • Meal Planning

  • Outings

  • Other Recreational Activities 

 

EL (Enhanced Living)

Opportunity Living is currently expanding our EL Program (Enhanced Living). The EL program is designed to work the children/young adults who are still in school and may have some specific needs. Our staff will work with the student according to the IEP, with the school, and also the AEA to see what needs we can meet and develop a plan to address certain areas.
